HOW DO I OBTAIN A REPEAT PRESCRIPTION ?
Requests may be made by post, by fax (01787  278628) or in person using the drop box in the outer lobby.  It helps us if you separate and keep the repeat prescription form, which is attached to your prescription, and return it to us with a tick against the medication you require, and return it to us. Also indicate whether you wish us to deliver your prescription to Clare Pharmacy, or whether you will collect it from the surgery. There are tick boxes at the top for this purpose. If you prefer us to post it to you, please let us have a stamped, addressed envelope.
 
Please order your prescription 48 hours (2 working days) before you need the medications.

Patients on long-term repeats will be asked to attend surgery at regular intervals for a medication check by the doctor.

 

HOW DO I OBTAIN TELEPHONE ADVICE ?

If you need advice from a doctor or nurse, please telephone in the morning. If the person you wish to speak to is not available, our staff will pass on your message but will require some indication of the nature of the problem.  For results of routine blood tests you may speak with the Duty Nurse between 2.00pm and 2.30pm.

HOW DO I MAKE AN APPOINTMENT ?
Please telephone 01787  277523 or call at reception. We try to keep to the appointment times given but on occasions emergency calls can prevent this. We offer a variety of appointments :from up to 2 months in advance to on the day. The latter are intended for urgent problems.
 
If you are unable to keep an appointment, PLEASE let us know as soon as possible so that the time can be offered to another patient.

If you think you might need an extended appointment, for example to discuss a problem of an emotional or personal nature, please mention this when booking.

HOW DO I OBTAIN A HOME VISIT ?

If you think a home visit is needed, please telephone 01787  277523  before 11.00am if at all possible. Our staff will require some indication of the nature of the problem including the name and address of the patient, or address where the patient is if they are not at home, and a telephone number where you can be contacted.
